Join us for the third installment of the Social Determinants of Health series focused on exploring barriers to accessing primary and specialty care. In order to improve population health and move towards value-based care, providers need to reorient and better incorporate upstream — social and environmental — factors into practice. This session will address key upstream health issues currently affecting the Greater Houston community.
